Southeast Geriatric Outreach Team

The Southeast Geriatric Outreach Team (SEGOT) is a comprehensive team of healthcare professionals who specialize in the care of seniors living in the community. Our healthcare team works together to promote independence and vitality for seniors through assessment, consultation, collaboration, and support.

The Southeast Geriatric Outreach Team may include the following inter-professional team members:

  • Consulting Physician
  • Physiotherapist
  • Social Worker
  • Registered Nurse
  • Occupational Therapist

Comprehensive Assessments

Working together with you and your family, our team will provide a comprehensive assessment to identify your health care concerns and provide helpful solutions.

These free assessments are available to frail seniors through in-home, virtual (Ontario Telemedicine Network- OTN) and/or telephone visits.

Assessments allow our healthcare team to identify any concerns and to connect with community supports for seniors who may be experiencing the following:

  • Recurrent falls
  • Multiple medication issues
  • Poor nutrition
  • Memory impairment
  • Difficulty managing day-to-day living
  • Social isolation
  • Multiple medical problems
  • Environmental challenges
  • Future planning

Referrals

A referral (form link below) is needed to be eligible to receive care by the GOT. Seniors can self-refer to the program or have a family member do so on their behalf. Referrals are also accepted from physicians, healthcare professionals, and community agencies.  Please note that referrals CAN ONLY BE SUBMITTED BY FAX at 905-201-5580.

Eligibility Requirements:

  • Possess a  valid Ontario Health card
  • Be 60 years of age or older
  • Live within the area served by the Southeast Geriatric Team (north of Steeles, south of Davis Drive, east of 404 and west of Durham 30)
  • Be experiencing a severity of health problems that would benefit from a comprehensive assessment.
