Adult Day Program

The Unionville Home Society Adult Day Program (ADP) offers a variety of activities aimed at promoting personal growth, exploration, and well-being. These include fitness sessions, creative workshops, reminiscing and discussion groups, music and movement activities, yoga, and cognitively stimulating activities. Participants enjoy a sense of community and connection while sharing a nourishing hot lunch and two snacks. All activities are designed to simulate independent living skills and enhance overall wellness.

The program is a good fit for a range of seniors from fully independent to those who require some aspects of physical and cognitive support. Prospective clients who meet the eligibility criteria must be referred to our program via Ontario Health atHome.

The Program

Our Facility

The Adult Day Program space was completely renovated in 2014.  With its own private entrance and drop off area, our facility offers approximately 1200 square feet of program space including:

  • A large bright main program room
  • 2 accessible washrooms
  • A full service kitchen
  • A separate therapy room
  • A fully furnished relaxation room

Program Details

In-person:

  • Programs that recognize and celebrate our community's diversity
  • A range of supervised therapeutic programs, daily fitness classes, creative art and music classes
  • Special events and presenters
  • Choice of a hot or cold lunch with two nutritious snacks (special diets available)
  • Facilitated transportation through York Region Mobility Bus

Additional Services

In addition to our traditional services, clients have access to optional services offered on a fee for service basis available during pre-set times.  These services may include:

  • Hairdressing
  • Podiatry

Our Staff

Our Adult Day Program is staffed by certified Personal Support Workers, Recreation Therapist and an Activity Professional. They are trained in Dementiability, First Aid and CPR and Safe Food Handler Certificate.

The program is designed to meet the diverse needs in our community.  Programming is delivered in English but language translation resources are used to ensure inclusion. We make every effort to accommodate language, cultural and spiritual preferences.  Our program is welcoming and our staff are accommodating. With a focus on meeting the needs of clients, we provide a flexible and innovative approach to programming.

UHS Adult Day Program Eligibility Criteria:

  • Must be able to feed themselves
  • Must be continent of stool
  • Must be able to follow a urinary continence routine
  • Must not be a high risk for falls
  • Must not be an active exit seeker
  • If they have any behaviors they must be manageable within our program
  • Must be able to independently sit to stand or be a one person guided assist

Other Details

The Unionville Home Society Adult Day Program  operates five days a week with in-person and/or virtual programming (subject to program fees). Access to virtual programs is for registered clients only.

PROGRAM FEES

In Person Full Day Program: $28.00 daily

Virtual Programs: $5.00 for group format

All referrals are made by Ontario Heath atHome through Ontario Health, Central Region.   Contact them by calling 1-(888)-470-2222.

TOURS – By appointment only and only for In-Person program

We welcome anyone to take a tour of our space and see our programs in session.  Please call the Manager of the Adult Day Program at 416-705-3825 to book or email us at dayprogram@uhs.on.ca

TRIALS – Only offered after referral is received by Ontario Health atHome:

In-person Program Trial is booked on the day of the vacancy, cost is $23.00 which includes one (1) nutritious snack and a hot lunch from 9:30 am to 2:00pm. Upon completion of the trial, UHS ADP will proceed with admission or help connect the client with a more appropriate service based on need.

Virtual Program Trial is one (1) zoom group session at no cost.
